DATES: April 16, 2023 from 1-3 pm. Location: Iroquois Park (near Amphitheater – park at Amphitheater and look for signs/tents)

- Pets must be healthy enough to receive a microchip
- Open to everyone
IMPORTANT NOTE ABOUT AGGRESSIVE PETS: We are a volunteer organization and we are not equipped to deal with aggressive pets. Any dog that has ever exhibited aggressive behavior must be properly restrained and wearing a properly fitted muzzle upon arriving at the clinic. Note: We have muzzles that can be borrowed, but owner must be able to place on and off their pet. However if the vet or staff feels that the pet is too anxious or aggressive, we may not be able to provide services.
REGISTRATION: Complete this form for EACH pet to register for one of our upcoming Microchip Clinics. Check your emails/texts prior to the clinic for your confirmation.
